[LIS-Forum] Book lost and replacement

Sir/madam, Pl. Give the solution for the below mentioned problem. In our college one faculty member is lost book and he is replacing the different title and author. Is it allowed the as per library rules. Can we accept it and what are the remarks we mentioned in accession register. Pl Give the solution.
