I quote Govt. of India General Financial Rules for this:
Rule no. 116(2) (ii): "Loss of three volumes per one thousand volumes
issued/consulted in a yerar may be taken as reasonable provided such loss
can not be attributed to dishonestyu or negligence on the part of Librarian.
Loss of a book of the value exceeding Rs.200 (Rupees Two hundred) and the
books of special nature and rarity shall invariably be investigated and
consequential action taken. All such losses will however be written off only
by a competent authority."
My comment: The rules have been drafted years before and the cost of a book
needs revision. This also makes it necessary that the library keeps perfect
statistics of borrowings, etc.

This is to ask expert opinion about the loss of books in libraries. I know of an Engineering College in Bangalore where they charge the Librarian for the loss of any books. The poor man tried his level best to convince the Management, but they are asking for rules and written documents to support his argument. Is there some provision which states that a loss of 4 books per 1000 can be considered or any rules from UGC or NCERT in this regard?.
